Waiver

WAIVER FORM - TERMS & CONDITIONS (Please read)

By completing this registration, I give my permission for my child to participate in all activities at Victory Hockey School and agree that Victory Hockey School, its owners and employees will not be responsible for any damage or accident (medical or dental) incurred while participating. I do waive, release, absolve and agree to hold harmless Victory Hockey School, its owners and volunteers in such instances. I hereby acknowledge that I have read the terms and conditions and certify that the applicant(s) is in good physical health. In the event of injury or illness, Victory Hockey School has my permission to obtain medical care for which I agree to be responsible. I give my permission for Victory Hockey School to use photos or video of my child.

Rowan's Law

The Ontario Government has enacted Rowan’s Law (Concussion Safety), 2018, S.O. 2018, c. 1 (“Act”). Ontario Regulation 161/19, the Act requires all sport organizations as defined in the Regulation (“Sports Organization”), which includes the Ontario Hockey Federation (“OHF”), to have a Concussion Code of Conduct. This Concussion Code of Conduct must require participants, as set out in the Act, to review the Ontario Government’sissued Concussion Awareness Resources on an annual basis. A participant is subject to a Concussion Code of Conduct for each Sports Organization a participant registers with.

Applicable age appropriate Concussion Awareness Resources are located at www.ontario.ca/concussions. The Concussion Code of Conduct and the appropriate Concussion Awareness Resources must be reviewed before you can register/participate in Victory Hockey School.

Refund Policy

Refunds can be requested 4 weeks prior to the camp start date for Holiday/March Break/Skills Camps.
For Summer programs, no refunds after June 15th for the upcoming summer programs.
There is a $40.00 administration fee for all cancellations.
