I am aware that GBC Awana activities include some risk of close contact with other children, and of the inherent possibility that my child may contract an illness from another child. I am also aware that there is slight risk of injury during games. I willingly assume responsibility for those risks as a condition of registering my child. I understand that if my child does not follow the rules set by GBC Awana and endangers his/her own safety and/or health, or the safety and/or health of another participant/adult, my child will be asked to leave the program and may not be allowed to return. *
