Juventus STL Commitment Form - 2021/2022

We are excited to have you be a part of Juventus STL family for the upcoming 2021/2022 seasonal year!

COMPLETION OF THIS FORM AND PAYMENT OF THIS NON-REFUNDABLE COMMITMENT PAYMENT THAT WILL BE APPLIED TOWARD FEES WILL LOCK IN YOUR CHILD'S ROSTER SPOT TO THE JUVENTUS STL TEAM FOR THE UPCOMING 2021-2022 SOCCER YEAR. Submission of this form confirms your intention to accept the roster spot offered for the 2021-20212 soccer year.

The commitment payment for 2021-2022 covers the following:

BIRTH YEARS 2003 - 2007 (High School): $325.00 this includes your annual $125.00 Juventus membership fee (pass thru to Juventus FC in Turin, Italy), as well as an initial $200.00 payment toward your child's annual club fees. This will reduce the balance due for the club fee by $200.00, leaving a remaining balance of $450.00.

BIRTH YEARS 2007 (8th grade) - 2012: $325.00 this includes your annual $125.00 Juventus membership fee (pass thru to Juventus FC in Turin, Italy), as well as an initial $200.00 payment toward your child's annual club fees. This will reduce the balance due for the club fee by $200.00, leaving a remaining balance of $600.00.

BIRTH YEARS 2013 - 2014: $225.00 this includes your annual $125.00 Juventus membership fee (pass thru to Juventus FC in Turin, Italy), as well as an initial $100.00 payment toward your child's annual club fees. This will reduce the balance due for the club fee by $100.00, leaving a remaining balance of $350.00.

BIRTH YEARS 2014-2017 (JUNIORS): $125.00, payment of the $125 annual Juventus Membership Fee (pass thru to Juventus FC in Turin, Italy) is your commitment payment. Juniors ID Sessions will start in late June.

PLAYERS ADDED FOR THE FALL 2021 SEASON AFTER 8/1/2021: $125.00, payment of the $125.00 annual Juventus Membership Fee (pass thru to Juventus FC in Turin, Italy) is your commitment payment.

One form can be submitted for ALL Juventus Academy Saint Louis players in your family and should be submitted as soon as possible.

Please note, if you need to setup special handling for your commitment payment, please outline what is needed in the NOTES section below. We offer Work-2-Play programs for families with a financial need to work off some/all of their club fees, and will always work with families to set up whatever payment arrangments are needed to make the financial side of soccer best fit your budget.
