Ciao Juventus STL Families,
For teams who have a staff coach as their HEAD COACH, there is an additional fee that is paid directly to the staff coach. This fee is the coach's compensation for their time, commitment, and expertise during your team training and games. It is not included in the club fee since not all teams have a staff coach.
We have setup this form as a way to help provide a reference for the separate fees that are due to the coach, as well as a means to communicate your preference on how to pay the fee to both the coach and the team manager.
If your team manager is collecting the fee on behalf of the coach, there is a place to note that below. However, all families with a player on a team with a staff head coach is asked to complete this form.
