Post Change of Program Period: For requests to drop NON-SIPA courses plesae submit a hard copy completed registration adjustment form in person to OSA.
THIS FORM SHOULD NOT BE USED FOR ANY TRANSACTION WHICH CAN BE PERFORMED VIA SSOL. Additionally requests which fall into any of the categories below will not be processed.
- Grade option change requests for full semester courses or for short courses which end prior to the pass/fail deadline. Requests to change to letter grade or pass/fail for these courses are submitted via SSOL
- Requests by non-SIPA students to register closed SIPA courses. Courses open to non-SIPA students are available for registration via SSOL. In the instance that an "open" course is not open via SSOL this form can be used.
