By clicking the "check mark" during registration, I, the undersigned, being 18 years of age or older, hereby agree to the following terms and conditions on behalf of the student(s) named above:

Liability Waiver: I understand and acknowledge that the Hanley School of Dance and the instructors of the Summer Intensive 2023 (hereinafter referred to as "the School") are not liable for any personal injuries, accidents, or incidents that may occur during the Summer Intensive 2023. I further understand that the School is not responsible for any loss, theft, or damage to personal property during the program.
Assumption of Risk: I acknowledge that participation in dance classes and activities involves inherent risks of physical injury. I voluntarily assume all risks associated with participating in the Summer Intensive 2023 and hold the School harmless from any claims, damages, or liabilities arising from such risks.
Student's Discretion: I understand that each student has the right to decline participation in any activity during the Summer Intensive 2023 without any obligation.
Media Release: I acknowledge that the School may produce promotional videos, photographs, and other materials related to the workshops. I hereby grant the School the perpetual right to use the registrant's photographs, video footage, and audio-visual recordings on social media platforms and for all purposes, including advertising and promotion of the Hanley School of Dance
Registration Fees: I understand that registration fees must be paid via e-transfer immediately following the completion of the registration form to secure a reserved spot. Any fees unpaid within 24 hours of completing the form will result in the loss of the reserved spot.
Registration Rules: I have read and understood the registration rules stated on the School's website. I acknowledge that the registration fee is non-refundable. I further understand that the School reserves the right to cancel the Summer Intensive 2023 for any reason.

To finalize your registration, please make an e-transfer payment to hanleydance@hotmail.com. We do not accept cash or cheque payments.

Scholarship winners are required to complete a separate registration form and should not be included on the same form as paying dancers. Each scholarship recipient must fill out their own individual form.
