Thurston County BOE Transmittal Form to Submit SUPPLEMENTAL INFORMATION FOR AN EXISTING PETITION
If your hearing has been scheduled, evidence must be submitted 21 business days prior to your hearing date.
Within the same timeframe, you must also provide a copy of this submission to the Assessor’s Office
(go to Submit Appeal Evidence under Online Tools on their website). 
NOTE: The BOE requires all electronic documents to be uploaded in PDF read-only format. If you are unable to upload your documents in PDF format, please do not submit this form. Instead, please choose one of the alternate options provided at the bottom of this form.
Please provide the following information about your existing BOE appeal(s):

Write Petition Number(s) on cover page(s) of your documents. This is particularly important if you are submitting separate packets of information for multiple petitions. However, if you have one packet of information that applies to multiple petitions, please indicate all applicable petition numbers on the cover page, and only one copy needs to be submitted
 
The BOE board members will not see the names of your electronic documents/photos, so please put titles, or applicable/explanatory notes on your actual documents if you wish them to be reviewed by board members prior to your hearing.
