Mesivta of TA: Application for Admission

Thank you for your interest in Yeshivas Chofetz Chaim - Talmudical Academy of Baltimore's Mesivta. We appreciate you taking the time to complete our application for admission.  In order for us to consider your application complete and reserve your son’s space in our program, please make sure to submit all of the following. If additional documentation is necessary you will be prompted by the application or contacted by the school.

  3. Current Immunization Records - At this time, this document can be a print out from the pediatrician's Electronic Medical Record System or a photocopy of the immunization record from your pediatrician's office.

    Please note: The Yeshiva requires all students to be immunized as required by law, and a claimed philosophical or religious objection to immunizations cannot be considered.  If, chas v’sholom, a student has a medical condition which prevents the administration of immunizations, please let us know immediately and the Yeshiva will evaluate if an exemption from the Yeshiva’s immunization requirement is warranted.

  4. Birth Certificate - A photocopy or scan of your son's birth certificate.

  5. Academic records - A copy of your son’s transcripts from his current school.  Please click here for a transcript request form.

  6. Applicant Evaluation Form - Form completed by student's current principal. Please click here for a copy of the form.
Current TA students only need to complete #1 and #2. The admissions office will get all of the other paperwork directly from the TA Middle School office.
